Course date (calendar date and year or term) Estimated enrollment in course Information regarding the person making this desk copy book request: Name Position School Department School Phone # Address City/State/Zip Office Hours Please email this form to books.publishing@sierraclub.org Desk copies will arrive via Media Mail. Please allow 2-3 weeks from the date of request.
